Following is a checklist of items to help you collect the documentation that you and your tax preparer will need:

Personal Information
- Social Security numbers for everyone who will be listed on your return as a taxpayer or dependent.
- Last year's state and federal tax returns
Income Information
- A W-2 form for each job at which you (and your spouse, if applicable) earned wages
- Form 1099-G if you had unemployment compensation and/or a State or local income tax refund
- Form 1099-SSA if you had Social Security Income
- Form 1099-MISC if you had other miscellaneous income
- Form 1099-INT if you had interest income
- Form 1099-DIV if you had dividend income
- Form 1099R Early Distribution of
Pension Funds or IRA Distribution
- Alimony received
- Jury duty pay
- Gambling, lottery, or prize winnings
- Scholarships
Child-Related Information
- Proof of childcare costs (cancelled checks or invoices)
- Childcare provider information (name, address, tax ID or social security number)
Home-Related Information
- Form 1098 showing mortgage interest
- Form 1099-S if you sold your home during the year
- Receipt for real estate taxes paid
Receipts for Other Expenses
- Student loan interest paid
- College education
- Personal property tax for automobiles, boats, or other personal property
